48 HOURS CANCELLATION POLICY

  • SFD has a 48-hour cancellation/rescheduling policy.
  • If an appointment is missed, canceled, or changed with less than 48 hours (2 business days) notice, there will be a $ 60 charge per hour.
  • Our practice realizes that there are many things that come up in people’s day-to-day lives. While truly sympathetic, the practice cannot absorb the financial responsibility of last-minute cancellations.
  • In fairness to all patients, this policy is in effect regardless of the reason for the cancellation.

DEPOSIT POLICY

  • SFD requires a deposit that will apply to copayment and/or balance in order to book/confirm appointments.

INSURANCE ESTIMATES

  • Insurance Estimates are just estimates.
  • Insurance companies do NOT provide 100% guarantee of payment
  • If your insurance does not make payment within 30 days, you will be responsible for the unpaid balance.

Southern Family Dental is in network for 10 insurance companies. We file all insurance paperwork for primary and secondary insurances and we will help you maximize your dental benefits.
